When learning task data is entered into the system a record should be kept of EVERY time the data is amended or added to - like a history of changes.

Learning Tasks - System Audit feature so you can see who/when has any changes on items. Also ability to see if Compass system makes any changes.
Saw an issue occur but no ability to see what has/hasn't occurs.
